Every time someone fills out your Gognito Form, Make will automatically create a new conversation in Help Scout. If the respondent's data does not exist yet in Help Scout, Make will also create a new customer. In order for this template to work, your Gognito Form should ask, among other things, for the respondent's email address, name and surname.
